Work Study Contract for Maria Gonzalez

Student’s Responsibilities:

 

Ms. Gonzales is expected to:

  • Arrive at work on time each day
  • Dress appropriately for a business casual environment (no jeans)
  • Be courteous and respectful of the staff and clients of the agency
  • Abide by all company rules
  • Maintain a record of the hours worked on the work study time card
  • Maintain a journal of questions she has had about her duties and the responses of her supervisor
  • Write biweekly reports for her instructor summarizing her experiences

 

Employer’s Responsibilities:

 

Ms. Smith will be Ms. Gonzalez’ supervisor. The employer’s responsibilities include:

  • Provide a safe work environment
  • Orientation of the student worker
  • Answer student questions regarding work assignments
  • Meet weekly with student to review performance
  • Verify hours worked and sign student’s work study time card
  • Submit time card to the school weekly
  • Evaluate student’s work performance biweekly and give copies of the evaluation to the student and to the student’s instructor

 

Instructor’s Responsibilities:

 

Ms. Jones is Ms. Gonzalez’ instructor. In addition to normal class interaction, Ms. Jones will:

  • Visit the student at the job site biweekly during student’s work hours
  • Review student’s journal and reports and provide feedback
  • Review work performance evaluations and provide feedback
  • Approve student’s work study time card and submit to Business Office for payroll processing
